When you arrive for your first visit at Shelley Kaur DDS, please be prepared to complete all new patient forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.

We will thoroughly present treatment options, fully explain our procedures and provide customized instructions for you to maintain your oral health. It is our goal to have you feel secure in the treatment of your dental needs

Non insurance:

If you do not have dental insurance, we are pleased to provide the following:

DENTAL MEMBERSHIP PLAN

2 Routine cleanings per year (includes fluoride for children)
1 Exams per year
Check up x-rays

Adults: $375       Children (12 and under) $275

15% off any additional needed treatment*

If you do not see your insurance plan on our list, please give us a call at (916) 939-6900 and we will gladly check for you.

We will bill your insurance and inquire about your coverage, co-pay but anything that is not covered by Insurance is the Patient's Responsibility. 

NOTE: If you have been recently been diagnosed with any type of heart condition, systemic infection, have or plan to have any type of surgery in the near future, please call our office at (916) 939-6900 so we can verify with your doctor if you need pre-treatment medications.
